DNA Kit Instructions
Welcome to The Art of Longevity.
In just a few weeks, you will receive your Signature Longevity Blueprint — a personalized synthesis of your DNA and lifestyle insights.
To ensure a seamless experience, please carefully follow the steps below after completing your DNA sample collection (instructions are included on the instruction sheet inside your box).
Kit Registration
Your DNA Kit must be activated before it can be analyzed.
- Visit longevity.lamaisonvalmont.com to access our secure platform operated by BMR, our partner laboratory in Italy.
- Select: “Register your kit”
- If you do not yet have an account, select “Create an account” and follow the instructions. Otherwise, log in.
- Once logged in, click “Register your kit” to continue.

Select the center where you purchased your kit.
- Please ensure this selection is accurate, as it allows your advisors to select customized experiences for you.
- Your center will never have access to your raw DNA data.
- On the same page, after selecting your center, enter your Kit ID. You can find it on the sticker inside your box, as well as on the outside under “Kit identification number.”
- Affix the same Kit ID sticker to the outside of the tube.

You will then be asked to review two consent checkboxes:
- Consent to the processing of genetic data (this consent is mandatory to proceed).
- Consent to the conversation of data for algorithm improvement (this consent is optional).
- If you choose to approve these consents, tick the boxes and click “Next.” You will receive an email containing a one-time password (OTP). Return to the page, enter the OTP, and click “Next” to confirm your selection.
- You will proceed to the Lifestyle Questionnaire.
Lifestyle Questionnaire
This step is essential. True longevity is not defined by biology alone — it emerges from the dialogue between your genetic predispositions, your habits, and your environment.
Take a quiet moment before beginning.
- Answer each question honestly and without judgment.
- Select the response on the scale that most accurately reflects your current reality.
- Just like your DNA sample, your detailed answers remain confidential. Your advisors will only receive an analysis — never raw data.
When completed, click “Send.”
Return Shipping of Your DNA Sample
Shipping is fully complimentary.
3.1 Request Your DHL Shipping Label
Send an email to shipping.longevity@evalmont.com with the following information for the ship-from address (the address from which you will send your DNA Kit):
- First name and last name
- Company name (if applicable)
- Full address
- ZIP / postal code
- City
- State (if applicable)
- Email address
- Phone number (including country code)
Within two business days, you will receive your DHL shipping label in PDF format by email.
3.2 Schedule Your DHL Pickup
Once you receive your DHL shipping label by email, please proceed to schedule a pickup with DHL.
- Visit mydhl.dhl.com and select your country.
- Click “Ship” and then “Schedule a Pickup.”
- When asked if you’re creating a new shipment, choose “No — I have a DHL Waybill Number.”
- Enter your 10-digit return waybill number (found on the DHL shipping label sent to you by email), along with your contact details.
- Select the day and time for pickup — DHL will come collect your package directly from your chosen address.
- Download and print your DHL return label (received by email).
3.3 Prepare Your Kit
- Ensure your Kit ID sticker is securely affixed to the tube.
- Place the sealed tube into the protective pouch provided.
- Insert the pouch back into your DNA Kit box.
- Remove the transparent adhesive envelope from the box before closing it — you will attach it afterward.
- Close and seal the box by removing the protective strip from the double-sided adhesive located along the inner flap, then press firmly to secure it.
- Place the DHL return label inside the transparent envelope, so that the barcode remains clearly visible.
- Stick the envelope onto the outside of the box.
- Ensure the DHL barcode is visible and positioned on a flat surface (you may fold the remaining paper if needed).
3.4 DHL Pickup
- Ensure your package is ready at the scheduled pickup date and time.
- Hand the sealed box directly to the DHL courier.
You can track your shipment at dhl.com using the tracking number on your return label.
What Happens Next?
Within approximately 3 to 4 weeks after DHL picks up your kit, you will receive an email informing you that your Signature Longevity Blueprint is available.
- Log in using the account created during registration.
- Download your report (available for 45 days).
Once your Blueprint is ready, we invite you to visit your The Art of Longevity center to complete your experience and begin your personalized journey.
